Has anyone ever played a game where there are two separate mafias in the same game? I was reading the mafia wiki and saw that possibility. Is it realistic?

Yes, the possibilities are endless with mafia games. If you can think it up, and get it balanced so that all teams have an equal chance of winning, you can do anything you want. I almost went with a third family in our Mob War game but decided against that. We have some games that have a third team, sometimes called a cult.

Of course, Cults tend to be survivalist, not killers.

The cult can generally recruit players into their team at the cost of having some of the more powerful abilities of the mafia roles. They are both anti-town.

And Ragnar did it before that in his weird mafia RPG hybrid. Not just two mafias but 3 or more are possible. In my hip hop elements mafia there was no actual town and it worked pretty well.

I've been thinking about one where there are multiple towns. Each town is balanced against the other with different roles having different point values (a vanilla = 1, and Doctor = 3 or something). When the game ends, the town with the most points still alive gets 1st, then the next most gets 2nd, and so on. This might cause the town to turn on each other to try to secure the higher place.

It's realistic and not unheard of. I played in three or four like that on this site. However, if you plan to design one I'd advise against it unless you've modded a few small games and at least one large game. The more complicated a game is, the harder it is to balance.
