How much would this change the history of the US? How would the world be different?

They explored this issue in an episode of Sliders (back in Season 1, when the series was actually good). Basically, it went like this:

Nobody else overthrew their monarchies (because the American Revolution hadn't inspired them to do so).

There was no Bill of Rights (because they were designed specifically to limit the powers of the government out of fear of a monarchy).

Nobody got to vote. The sheriff of San Francisco (who was Professor Arturo's double) made all the rules.

They didn't explore this, but I reckon there'd be more fighting over religion. Without the Separation of Church and State, religion would be a political issue and people would fight wars over it just like they did in Europe. In fact, this would probably divide the US into several monarchies, each trying to enforce their beliefs onto everybody else - which, come to think of it, wouldn't be that different from how things are now.
