How do you differentiate between a person having delusions about an angel and a person seeing a real angel?

Anyone seeing an angel is delusional.

There is nothing delusional about seeing a real angel.
It is however delusional when you tell someone you seen a real life actual factual, really real life like , actual living, Angel . And his name is Terrence,
What I'm trying to say is. If you see angels , keep it to yourself.
The Same should occur for people with a god..
Having or seeing a god is the exact same as having a angel named Benjamin.
We felt what it was like believing, having a Santa. It's all the same.
I would name my angel , ERIC. Yep . Eric

One is an idiot, the other is a cool guy called James:

Great question but these are the types of questions theists don't engage in so as to not expose the complete magnitude of their lunacy.

The theists that actually think about these types of questions stop being theists. That is what happened to me. To remain a theist, they must avoid analyzing such conundrums.

Well, most of the people in the Bible were absolutely terrified when they saw one, so that might be a good place to start.

I am not sure how good of a start it would be, as those (recently) whom have claimed religious epiphany and claimed to have seen an angel haven't really mentioned heart-gripping fear.

Something else to consider, the Gods of some pantheons were straight up freaks. Winged lions, falcon headed humans, multiarmed elephants... I haven't heard many accounts (historically) from those religions that put 'fear' with regards to their patron deity's appearance.
Here we have an advocate for Islamic arranged marriages demonstrating that children can consent to sex.
