FOIA Request forms are easy enough to use as long as you read the instructions for the agency you want to request information for. If a citizen can file taxes or fill out a FAFSA for an educational grant, the FOIA request is in a similar vein and is not too difficult to maneuver through.
The FOIA request form is easy enough for the average citizen to fill out. They do make some of these forms very hard to understand and read making it impossible for people to go forth and fill the forms out correctly. I think it should be screened by all different people to see if it is right.

There is no form for a FOIA request. FOIA requests must be written and they must describe the information you are seeking and the format you wish to receive it in. The request must go to the correct party in order for it to be considered. I think we would be better off with a form, but thus far none have been used.
